When coming to see us for your first appointment, here is a list of the things that you should bring with you:

- Your insurance information

- Insurance information booklet

- A list of medications you are taking and allergies, if any

- Contact information for your family doctor

Payment Information

-     You can pay for our services using cash, cheque, debit, VISA, or MasterCard.

-     If you have insurance coverage, we offer direct billing! This means that after your appointment you need only pay any amount not covered by your insurance provider.

-     You are responsible for payment on the day of your visit.
